Have both sides agreed?

This form records that the landlord and tenant have both freely agreed to end the tenancy. It isn’t a notice to end tenancy, and nobody has to sign it. A tenant who signs may give up compensation they’d otherwise be owed; the form says so, and both sides should read it before signing.
